Dry, Flaky Scalp on a Bald Head: What Helps

Flakes and tightness don't stop when the hair does. How to tell dry skin from dandruff, and a simple routine that clears most cases.

Not medical advice. This is general information. For anything involving prescription treatment, a changing mole, or a scalp condition, talk to a doctor or dermatologist.

Many men assume that once the hair is gone, so are dandruff and a dry scalp. They aren’t. The skin is still there, and the oil glands still work. Shaving and sun can make flaking worse.

Dry skin or dandruff?

Dry scalp Dandruff
Flakes Small, white, dry Larger, oily, yellowish
Skin Tight, sometimes itchy Itchy, sometimes red and greasy
Typical cause Cold, low humidity, hot showers, shaving, sun A reaction to a yeast that lives on skin (seborrheic dermatitis)
What helps Moisturizer, gentler washing Medicated shampoo

Many men have a bit of both. If you’re unsure, start with the moisturizer routine, and add a medicated shampoo if it’s still flaking after a couple of weeks.

A simple routine

  1. Wash with lukewarm water, not hot. Hot showers strip natural oils.
  2. Use a gentle cleanser on your scalp. Harsh, fragrant soaps dry it out.
  3. Pat dry and moisturize while the skin is still slightly damp. A plain fragrance-free cream or lotion is enough.
  4. Exfoliate lightly, once a week at most. A soft washcloth beats scrubs.
  5. Add a medicated shampoo two or three times a week if you suspect dandruff. Leave it on for a few minutes before rinsing, as the label directs.
  6. Wear sunscreen daily. Sunburn causes peeling that looks like flaking. See our sunscreen guide.

Things that make it worse

When to see a doctor

Flaking that comes with thick, scaly red patches, bleeding, a spreading rash or intense itching doesn’t always mean ordinary dandruff. It could be seborrheic dermatitis that needs a prescription, psoriasis, or eczema. A dermatologist can tell, and the right treatment is usually simple.
